Important Information

  • Our schedule is available up to 20 days in advance. If you’d like to schedule an appointment beyond that time, please check back when your preferred appointment date is within the 20-day window.
  • Before scheduling an Intake Assessment with our therapists, please ensure you have read our Office Policies and Insurance and Fees
  • Within 48 hours of scheduling your appointment, CHARM Health will email you instructions for registering for our patient portal. If you do not receive an email, please check your spam folder. Please register for the portal as soon as possible. In addition, check your email for additional instructions and complete any requests.

This is a 60-minute intake assessment with the therapist you select. This appointment will be billed to you or your insurance if applicable. You must be physically located in Oregon for this appointment and all future appointments.
